Once paperwork is completed and reviewed, the evaluation appointment can be scheduled right away

2 payments of $600
First payment of $600 provides access to intake paperwork and scheduling
Once paperwork is completed and reviewed, the evaluation appointment can be scheduled.
Second payment of $600 must be made before the written report and feedback session are provided
Payment Plan:
3 payments of $400
First payment of $400 provides access to intake paperwork and scheduling
Second payment of $400 must be made at or before the evaluation appointment
Third payment of $400 must be made before the written report is released and feedback session is scheduled.
The third payment of $400 must be made before the report is released or feedback appointment scheduled.
Additional Policies
Payment plans are a financial arrangement only and do not function as pay-per-session services.
Once the evaluation interview has occurred, the remaining balance remains due according to the selected payment plan.
Written reports and feedback sessions are not provided until the evaluation balance has been paid in full.
If a client decides not to move forward before intake paperwork has been submitted, payments may be refunded minus Stripe or credit card processing fees.
Once intake paperwork has been submitted, reviewed and an evaluation appointment has been scheduled, the initial payment becomes non-refundable.
Once the evaluation interview has occurred, no refunds are available and the remaining balance remains due according to the selected payment plan.
